Position Summary:

To direct the planning, development, implementation and evaluation of recreational, physical, social, intellectual, and emotional programs that enrich the lives of resident/tenants in accordance with resident/tenant's assessments and care plans/service plans. Such duties must also be in accordance with federal, state and local standards, guidelines, regulations that govern Bethany Lutheran Health Services and as may be required by the Administrator to ensure that the highest degree of quality care is maintained at all times.

Qualifications:

  • No record of abuse.

  • One-to-two years’ experience is preferred.

  • Must be able to read and communicate the English language in an understandable manner.

  • Must be able to use standard office equipment.

  • One-to-two years’ experience in leadership role.

  • At Bethany Home C.N.A required at Bethany Heights recommended.

  • Activities Director Certification or able to obtain certification in one year

  • Certified Relocation & Transportation Specialist (CRTS) Certification or able to obtain certification in one year.

Essential Job Functions:

  • Monitor resident/tenant attendance, mood, behavior, and degree of involvement regularly so that activities are adjusted for quality and effectiveness.

  • Create new, safe and stimulating activities for residents/tenants monthly. Ensure all activities are implemented and supervised according to the schedule.

  • Encourage resident/tenant, family and staff participation in all activities.

  • Schedule residents/tenants for activities and assist residents/tenants to and from activity location including arranging of transportation.

  • Maintain accurate resident/tenant records (daily participation sheets, resident/tenant assessments, progress notes, etc.) and other records as assigned.

  • Always maintain confidentiality of resident/tenant care information

  • Participate in community related activities in the interest of the resident/tenants and facility.

  • Plan, develop and implement a care plan/service plan and assessment procedures as needed.

  • Attend scheduled staff meetings/in-services, weekly management meetings and facility sponsored events.

  • Establish and maintain good working relationships within the activities department, and other departments within facility

  • Keep abreast of current federal and state regulations as well as professional standards and enforce all written policies and procedures. Make recommendations for change as appropriate to the Administrator.

  • Coordinate all services with other departments to ensure efficiency, timeliness, and convenience to residents/tenants, staff, and visitors.

  • Review departmental policies and procedures, at least annually, and participate in making recommended changes.

  • Collect, retain and disseminate information to the resident/tenants, staff, and visitors concerning activities, local events, and trends.

  • Participate in discharge procedures for all resident/tenants as needed

  • Review and participate in the development of the plan of correction for any deficiencies noted during survey inspections.

  • Interview family members for current resident/tenant activity levels and modify as necessary or directed.

  • Forecast the needs of the department on a annual basis, prepare and present the budget to the Administrator for review, and assist staff in maintaining the budget throughout the year. Budget should include any capital improvements, and any other necessary expenditures based on estimates of experience. Notify the Administrator immediately of any necessary changes and adjust as required or requested.

  • Delegate a responsible staff member to act on your behalf in your absence and assign daily duties to each staff member.

  • Attend at least ten hours continuing education per year.

  • Act as Resident/Tenant Council facilitator as necessary.

  • Actively participate in QAPI activities and meetings and completing critical pathways.

  • Other duties as assigned by the Administration/Housing Director.

Physical Requirements:

  • Must be able to move intermittently throughout the day.

  • Must be able to push, pull, move and/or lift a minimum of 50 lbs. to a minimum of 3ft.

  • Must be able to assist in the evacuation of resident/tenants in an emergency situation.

  • Must be able to kneel, stoop or bend to assist resident/tenants.
