PAMHC is seeking a full-time Case Manager for our Denison/Carroll locations. Periodic travel required for in home and community-based services for clients or to other office locations as assigned. Position is an average of 40 hours per week Monday – Friday, with occasional evenings as needed to meet with clients.

Pay Range:

$23.77 to $26.24 depending on experience

Position Summary:

Each Case Manager will be responsible for working with clients and other Plains Area Mental Health Center team members to enhance treatment through client engagement. Case Managers are responsible for the engagement with clients, overseeing the agency treatment plan, and completing assessments and updates with the client directly to meet agency compliance timeframes. Case Managers will find appropriate resources to address Social Determinates of Health for clients and may assist with urgent needs such as housing, food, etc. Case Managers will manage the communication of court ordered clients, working to engage with clients and coordinate care between providers. This position will also be responsible for collaborating with various entities throughout the catchment area including, but not limited to, emergency departments, urgent care clinics, inpatient medical health units, and inpatient behavioral health units.

Requirements:

  • Valid Driver’s License and Insurance

  • Bachelor’s degree in human services field, with one-year experience working with persons with serious mental illness

  • Licensed RN in the state of Iowa with at least three years’ experience working with persons with serious mental illness

Preferred Qualifications:

Bilingual a plus, but not required
