Job Description

POSITION SUMMARY: Assist with community health programs, including the WIC program to fulfill program objectives.

ROLE AND RESPONSIBILITIES

  1. Comply with federal, state, and agency policies and procedures
  2. Maintain cooperative, collaborative relationships with state agency staff and community health partners.
  3. Provide quality WIC services that comply with federal and state guidelines; assist with set-up and tear-down at mobile clinics, perform client intake, complete nutrition assessments, provide client education and referrals, and issue food benefits.
  4. Complete quality documentation in the WIC computer system. Provide breastfeeding education and support, as needed
  5. Complete annual training and continuing education requirements.
  6. Participate in all required meetings and trainings
  7. Adhere to all CNCAP policies and procedures

QUALIFICATIONS

  1. Registered Nurse or Licensed Practical Nurse
  2. Valid driver's license

SCHEDULE: Monday - Thursday

REPORTS TO: Executive Director

JOB STATUS: Full-Time or Part-Time

PAY: LPN: $18.89-$20.42/hour (depending on qualifications)

RN: $22.21-$23.49/hour (depending on qualifications)
