Partnership Community Health is a local not for profit that connects people with healthcare, dental, and behavioral health by breaking down barriers to coverage. The Registered Nurse, As part of a team-based approach to patient care and under the direct supervision of the Primary Care Director, provides high quality, culturally appropriate care to a variety of patients throughout the lifespan. Utilizing the nursing process (assessment, diagnosis, planning, implementation and evaluation) to identify and manage health problems. This position is 32 hours/week.

Qualifications
- Current License to practice nursing in the State of Wisconsin; BSN preferred.
- Bilingual (English/Spanish) helpful.
- Able to achieve and maintain BLS certification.
Key Accountabilities
- Triage of telephone, walk-in, MyPartnership and scheduled patients. Maintain thorough documentation of all encounters in the electronic medical record and medication distribution log.
- Communicate patient test results per provider instructions.
- Demonstrate a sound working knowledge of basic nursing principles and professional ethics.
- Assist the Primary Care Director in the coordination of patient flow and providing ongoing education for support staff.
- Possess well-developed communication and interpersonal skills to allow for effective functioning as a member of a multidisciplinary team within a cross- cultural environment.
- Respond to communication from other sources such as pharmacies, labs, hospitals, PatientPing from other physician offices and manage their issues.
- Monitor and respond to patients through electronic communication using My Partnership.
- Maintain the Community Emergency Prescription Drug fund in a cost-conscious manner that best aids the patient.
- Assist with follow up, vouchers and referrals.
- Perform or assist with emergency or standard procedures and treatments such as injections, blood pressure check, and co-management with the providers of the International Normalized Ratio (INR)
- Demonstrate a commitment to using information technology to facilitate optimal organizational outcomes.
Perform miscellaneous nursing duties as assigned.
